SJA Admission Interest Form

In order to register your child, please fill out this online application and bring an ORIGINAL of the following documents to school:

• Birth Certificate
• Social Security Card (or Passport)
• If Catholic, a copy of the child’s Baptismal Certificate
• Complete Immunization Record
• Proof of Residence (utility bill with your address)
• Proof of Guardianship (if this situation exists)
• Grades 1-8: A copy of the most recent report card and a copy of the most recent standardized test scores

If you are registering a student who is currently enrolled in Catholic school in the Diocese of Brooklyn and Queens, email [email protected] directly for a streamlined application process.

 Children for Pre-K 3 should be toilet trained. 

The registration fee is $250 per family, cash or check, and paid at registration.
